We can separate wireless services without notifying anyone on the account beforehand. It only takes a few steps. We'll ask about you and your wireless service, then have you upload required documents.

Verify number

To verify your number, you can sign in to myAT&T, get a verification code, or provide a number you recently called.

Provide documents

Upload accepted documentation so we can process your request.

Review your details and send your request. We'll process it in the next two business days. if we need more info, we’ll contact you using the method you provide.

Accepted documents

You or your designated representative must provide supporting documents. See examples below.
  • Required: Documentation that verifies an alleged abuser who uses a line on the account has committed, or allegedly committed, a covered act. Accepted documents include:

    • Signed affidavit from a medical professional or social worker
    • Police report or court record
    • Any other official record that documents the covered act
    • Self-attestation form (New York residents only)
  • Optional (dependent lines): A signed and dated affidavit that shows lines you want to separate belong to someone in your care or the care of the survivor you represent

  • Optional (designated representative): A statement from the representative who helped complete this form on behalf or the survivor

You can call Customer Service at 800.983.4428. We’ll connect you to specially trained representatives who can handle your requests in a secure and confidential way. They can help you separate your wireless lines due to intimate partner violence or abuse. 

  • You'll need to provide us with accurate, basic information about your situation and the phone numbers involved.
  • Some states may require you to provide supporting documentation from a court, law enforcement, health care provider or social worker.
  • We can't always accommodate every situation but will endeavor to minimize the cost and burden of this process whenever we can.

 

Lifeline is a federal program that lowers the monthly cost of service for qualifying low-income customers. It provides a discount on qualifying monthly wireless plans. If you aren’t eligible under the regular requirements, you may be eligible for an emergency benefit for survivors for up to six months.
